JOINT AFFIDAVIT OF NEAREST RELATIVES

We, RAMONITA V. ANGELES, of legal age, married, Filipino Citizen, and with postal address 1660
Lancer St., Culdesac, Brgy. Sun Valley, Parañaque City and FLORDELOUNA V. GAYONDATO, of legal age,
married, Filipino Citizen, and with postal address at 4035 Culdesac Rotonda, Brgy. Sun Valley, Parañaque City,
after having been sworn in accordance with the law hereby depose and state:

1. That we the nearest relative of the deceased RAMILO M. VILLAMOR who died last December 23, 2022
at Sindalan, San Fernando, Pampanga;

2. That the decedent is a member of Social Security System (SSS) with SS no. 02-1441323-4;

3. That the decedent has a long-time live-in partner: CAROLINA Q. DAYRIT who he has been living since
1983;

4. That they are blessed with four (4) children, namely;

5. However, as his family processing his Death Claims at SSS, they found out that in Certificate of Live
Birth of his youngest: RICH D. VILLAMOR, the decedent wasn’t able to signed his
acknowledgement/admission of paternity to the child. A copy of the said Certificate of Live Birth hereto
attached to this affidavit for reference;
6. That we hereby declare that RICH D. VILLAMOR is a legitimate son of the decedent and dependent for
financial support prior to the death of deceased member and requesting all the benefits to induce to him
and his family;

7. Finally, we are executing this Joint Affidavit to attest to the truth and veracity of the foregoing set of facts
without any promise or reward or any monetary consideration but solely for the purpose of declaring the
above-mentioned events were true and factual.

IN WITNESS WHEREOF, we have hereunto set our hands this 1st day of February, 2023 in Parañaque
