AFFIDAVIT OF TERMINATION OF EMPLOYMENT

I, JOYER DISTOR FONTANILLA, Filipino, of legal age,

married, with residence and postal address at 3370 Dimasalang
Ext., Corner, Bagong Buhay St., Baclaran, Parañaque City,
accordance with law, hereby depose and state that:

1. I am bona fide member of the Social Security System

(SSS) with assigned SSS Number 03-9313095-9;

2. I was an employee of Panasonic Avionics Services PTE

LTD with office address at 65 Airport Blvd., #01-03
Changi Airport T3, Singapore 819663 from September 1,
2012 to August 31, 2020;

3. I was terminated from employment effective August 31,

2020 due to retrenchment;

4. I cannot secure a Notice of Termination from my

employer; and

5. I am executing this affidavit to the truth and veracity of

the foregoing statements and to support my application
for unemployment insurance or involuntary separation
benefit.
