I, EDNA B. SAMBILE, of legal age, Filipino Citizen, and a

resident of 19485-B, Sampaguita St., Matatdo Homes, Parañaque
City, after having been duly sworn on oath in accordance with law
hereby voluntarily depose and say, THAT:

1. That I am the biological mother of Rhena B. Del Carmen

who is the owner, of the certain business name CHBR
Water Refilling Station which is located at 19485-C,
Sampaguita St., Matatdo Homes, Parañaque City;

2. That she (Rhena B. Del Carmen) declare to state transfer

her ownership to me (Edna B. Sambile);

3. That she also, hereby declare and state that she

transferring the operation and administration of the above-
mentioned;

4. That I am executing this affidavit to attest to the

truthfulness and honesty of the foregoing facts and for all
intents and purposes it may serve.
