Laguna, after being sworn in accordance with law, depose and state:

1. I am applying for the position of Branch Clerk of Court V in Regional Trial

Court of Parañaque City, Branch 296;

2. I have no relatives within the third civil degree of consanguinity or

affinity who is an incumbent employee in the subject branch;

3. Should the Office of the Court Administrator of Supreme Court ascertain

that I have relatives in the subject branch, the Office of the Court Administrator reserves
the right to cancel the processing of my application;

4. I understand that should I be employed, any misrepresentation on the

matter is a ground for an administrative case and termination;
