AFFIDAVIT

I, JOHN REY A. PARANTAR, of legal age, married and with

address at Vista Verde, Upper Canitoan, Cagayan de Oro City,
after having been duly , sworn to in accordance with law do
hereby depose and say:

1. That I am filing for application for installation of water

services through the Cagayan de Oro City Water District (COWD);

2. That said application is under account number 151B-12-

6605/ 68358 previously under the name JOSE G. PARANTAR;

3. That I am requesting that the payable balance in the

amount of Php 5,000 be forwarded to my name and I promise to
the same by adding the amount of Php 500.00 for every monthly
billing until the same will be fully paid;

4. That the said transfer is necessary for the installation of

water services and because the above-mentioned previous owner
could no longer pay the same;

5. That I execute this Affidavit to attest the truth of all my

foregoing statement and for whatever purpose it may serve;
