AFFIDAVIT OF UNDERTAKING

I, ALINAIR P. MAMAO, of legal age, married and resident of Cagayan De Oro

City, after being sworn to in accordance with the Law, hereby depose and say, that:

1. I am the Chairman of the Board of New Mindanao Transport Service

Cooperative, the Office of such is located at B13 L29 Johndorf Subdivision,
Barra, Opol Misamis Oriental;

2. That I am in-charge of causing the preparation and submission of our

application at LTFRB Region 10;

3. However, due to the arduous and numerous requirements needed for our
application, I may have overlook properly some documents ;

4. That I am securing this affidavit and promise that any lacking requirement
after submission will be submitted;

5. Failure on my part to comply with the foregoing obligation shall lead to

the dismissal of our application; and

6. This Affidavit of Undertaking is executed to attest to the truth of the

foregoing statement, and before affixing my signatures below, its contents
were read and explained to me in the vernacular and the same were fully
understood by me.

IN WITNESS WHEREOF, I have hereunto set our hands and affixed our
