AFFIDAVIT TO AMMEND PARAGRAPH 4 ON THE

PETITION FOR CANCELLATION OF APPROVED PLAN

That I, LAURENCE DELIM of legal age, Filipino citizen, married to

Emelita Delim and resident of Panicsican, San Juan, La Union, do hereby
depose and state:

1. That I am the affiant of a Petition for Cancellation of

Approved Plan, duly notarized by Atty. Jose B. Ellazar
under Doc.No.: 96; Page No.: 20; Book No.: III; Series of 2019,
dated August 6, 2019;

2. That said Petition was submitted to the office of the

Department of Environment and Natural Resources (DENR),
Regional Office I, San Fernando City La Union and was
submitted also in Regional Trial Court, Branch 67, Bauang,
La Union;

3. That the statement of Paragraph 4 is hereunder, to quote:

“4. That moreover, I obtained the property as innocent purchaser

for value, having no knowledge of any irregularity, defect at the
time I signed the Sale;”

4. That the statement shall be amended by, to wit:

“4. That moreover, I obtained the property as innocent purchaser

for value, having no knowledge of any irregularity, defect at the
time I purchased the Sale;”

5. That this amendment was done to correct the word “signed”

to “purchased” for reason that the word “signed” was
inadvertently typed;

 6. That the amendment and/or correction will be part of the
integral statement of the Petition for Cancellation of
Approved Plan, hence, this Affidavit will serve as essential
and request that this will be placed on records;

That I am executing this Affidavit to attest to the truth of all the

foregoing facts.
