AFFIDAVIT

I, ANTONIO V. SANTOS, of legal age, widower, Filipino, and presently

residing at Jimenez Subdivision, Brgy. Gabu-an, Roxas City, Philippines, under
oath, depose and say:

1. I am the owner of A. SANTOS NEWSPAPER & MAGAZINE, which is

also its DTI business name, situated at Poblacion VIII, Roxas City,
Capiz, Philippines;

2. The said business is a holder of DTI Certificate No. 05259082 valid

from March 8, 2018 until March 8, 2023;

3. This business when it operated for more than three (3) months since
March 8, 2018 or until sometime in June, 2018, it did not gain as
expected and I only incurred losses in it in which I could no longer
maintain its needed capital, therefore, I decided to stop or close its
operation;

4. In view of the above reasons, I am requesting the Office of the

Department of Trade and Industry, Roxas City to close and/or
cancel the above-mentioned business name;

5. I am executing this Affidavit to attest to the truthfulness of the

foregoing facts stated herein and for any legal purposes it may serve,
and further to request the Office of the Department of Trade and
Industry to close and/or cancel the above-mentioned business
name;

6. This Affidavit is not intended to defraud creditors since I have no

outstanding and pending financial obligation with any lending
company or institution, but this is executed merely for the purposes
above-stated.
