AFFIDAVIT

That I, ALFREDO VILLANUEVA, of legal age, married, Filipino,

and a resident of Barangay Gabu-an, Roxas City, after having been duly
sworn according to law, depose and say:

1. That I am applying for a firearms license before the Firearms

and Explosives Division at Camp Crame, Quezon City;

2. That I am a faithful and law-abiding citizen of the Philippines

and I do not have any pending cases, criminal, civil, and
administrative, nor involved in any of the aforementioned cases,
before any courts or tribunal anywhere in the Philippines;

3. That I am executing this Affidavit to attest to the truthfulness of

the foregoing and for any other legal purpose.
