JOINT AFFIDAVIT

We, HEMSIL ESQUILLO and SHEILA GUNDRAN, both of

legal age, both married, Filipinos, and residents of Barangay Barra, Roxas
City, under oath, depose and say:

1. We know personally IMELDA C. GOTERA - MAGALLANES, she

being our close family friend for so many years already;

2. We also know for a fact that her true and correct middle name as
what is appearing in all her public and private documents is Gotera
and the initial “C” which stands for Cubita is her maiden middle
name;

3. However, in her SSS records, her middle name was erroneously

entered as Cubita instead of the correct one, “Gotera”;

4. The names Imelda C. Gotera – Magallanes, Imelda Cubita

Magallanes, and Imelda Gotera Magallanes, refer to one and the same
person;

5. We are executing this Joint Affidavit in our own free will to attest to
the truthfulness of the foregoing facts stated herein and for any legal
purposes it may serve.

6. We have no intention of defrauding anybody or any institution but

only for the purpose above-stated.
