AFFIDAVIT

I, Lauriza D. Marquez, of legal age, Filipino, with office address at Level 7 Cyberpark Tower 1
Araneta Center Cubao, Quezon City after having sworn in accordance with law, hereby depose and state
the following:

1. I am the Collections Manager of Robocash Finance Corporation (RFC), a financing company duly
organized and existing under the laws of the Republic of the Philippines, with office address at
Level 7 Cyberpark Tower 1 Araneta Center Cubao, Quezon City;

2. On 16 September 2019, Edna Ibanez Elevado borrowed the sum of Five Thousand Five Hundred
Pesos (P5,500.00) from RFC, as evidenced by the Promissory Note, Borrower’s Undertaking,
Disclosure Statement of Loan/Credit Transaction and Cash Voucher dated 16 September 2019.

A copy of the Promissory Note, Borrower’s Undertaking, Disclosure Statement of Loan/Credit

Transaction and Cash Voucher all dated 16 September 2019 are hereto attached as Annexes “A”,
“B”, “C”,and “D” and made integral parts hereof.

3. Under the Promissory Note and Disclosure Statement of Loan/Credit Transaction dated 16
September 2019, Edna Ibanez Elevado undertook to pay RFC as follows:

a. Nine Thousand Six Hundred Twenty Five Pesos (P9,625.00) payable upon maturity or on
16 October 2019;
b. That there is one-time late payment charge of Six Hundred Pesos (P600.00);
c. That there is a daily late payment interest charge of 3% from 1 st to 30th day of non-
payment;
d. That on the 31st day until full payment of loan, the daily late payment interest charge will be
2%;

4. In all, Edna Ibanez Elevado owes RFC the sum of Twenty Seven Thousand Two Hundred Fifty
Pesos (P27,250.00) as of 25 May 2021, inclusive of the unpaid balance of his obligation with
agreed interest on the overdue account continuously being incurred. It needs no mention that the
said sum is now due and demandable.

A copy of the computation is here to attached as Annex “E” and made integral part of hereof.

5. Via a demand letter dated 25 May 2021, RFC sent Edna Ibanez Elevado a final demand to pay. This
demand letter was received by Edna Ibanez Elevado on 28 May 2021.

Copies of the Letter and Affidavit of Service dated 28 May 2021 are hereto attached as Annexes
“F”, and “G” and made integral parts hereof.

6. To date, Edna Ibanez Elevado has failed to respond to the aforesaid final demand letter.

7. It is for these reasons that RFC is constrained to institute the instant complaint in order to recover
from her the sum she borrowed.

8. I execute the foregoing Affidavit as duly authorized representative of RFC and as part of the
requirements in filing a small claims complaint before the courts of justice.
