AFFIDAVIT

I MARITES C. NIVERA, of legal age, Filipino citizen, and a resident of Duramos

Drive, Barangay Pasonanca, Zamboanga City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and say that:

1. I am the cashier of Philippine Regulatory Commission Region IX Workers Multi-

Purpose Cooperative;

2. PRC R-IX Workers Multi-Purpose Cooperative issued a memorandum dated 10

June 2021 with subject WRITTEN EXPLANATION ON THE UNAUTHORIZED
OFFICIAL RECEIPTS BOOKLET FOUND ON YOUR POSSESSION LAST
JUNE 7, 2021 AND WAS ISSUED TO TRANSACTING CLIENTS;

3. In response to the memorandum, I vehemently deny to be in possession of the

unauthorized official receipts, further, I issued receipts in the ordinary course of
business;

4. I have not in any way misused or misappropriated any receipts of the

cooperative;

5. I am executing this Affidavit to attest to the truth of the foregoing fact and to
comply with the memorandum issued by PRC RIX Workers Multi-Purpose
Cooperative and for whatever legal purposes it may serve;
