AFFIDAVIT OF LOW INCOME

I, FLORENTINO D. TOLENTINO, Filipino, of legal age, widower, and a

resident of Ilang- Ilang St., Kabacan, Matina, Davao City, after having been duly
sworn to in accordance with law, hereby depose and say:

1. That I am a PUJ operator;

2. That I only have one PUJ and for the year 2009am a widow and I only
have two adult children but are jobless;

3. That I am executing this affidavit to apprise the authorities concerned

on the veracity of the foregoing facts and circumstances and for
whatever legal purpose this may serve.
