AFFIDAVIT OF LOSS

I, JOEL R. DAGOT, Filipino, of legal age, married and a resident of

Brrgy. Garang, Tangub City, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state, THAT:

1. That I graduated my Doctor of Education at Misamis University,

Ozamiz City;

2. As such, I was issued a Diploma by the said university;

3. Sometime last year, 2020, upon checking in the place where I

used to keep the above-mentioned document, the same was no
longer there;

4. I have made diligent efforts to find the same, but, proved futile;

5. As such, I am executing this Affidavit of Loss to attest to the truth

of the foregoing, and for the issuance of new Diploma and for
whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 1 st day
