AFFIDAVIT OF EXPLANATION

I, JINKY MARY S. COLON, 43 years old, Filipino, single, and a resident

of Purok CE Maurillo, Zone 4, Mangga Visayan Village, Tagum City, Davao del
Norte, Philippines after having been duly sworn to in accordance with law, do
hereby depose and state THAT:

1. I am the mother of JOEL JR. COLON AÑONUEVO (JOEL JR. for brevity)
who is 18 years old and a Senior High student enrolled at Tagum
National High School;

2. My son, JOEL JR. COLON AÑONUEVO, Certificate of Live Birth issued

by the Philippine Statistics Office indicates that his name is JOEL JR
COLON with no Last name written/indicated in the same document;

3. JOEL JR., is has been using the last name of his father JOEL
LLADONES AÑONUEVO since he started studying in school;

4. JOEL JR. COLON and JOEL JR. COLON AÑONUEVO is one and the
same person;

5. JOEL JR., intends to use the surname of his father, JOEL LLADONES
AÑONUEVO, however he has to file a petition before the court to use
the surname of his father;

6. He is on the process of collating the documents needed for the filing

of the abovementioned case before the court of Metro Manila as it
has jurisdiction to hear and try the case;

7. Since the proximity of the court is far from his residents, he intends
to file the case after his graduation;

8. I am executing this affidavit to attest to the truth of the foregoing

statement and for whatever legal it may serve.
