I, JOHN MICHAEL NEPUMUCENO, of legal age, Filipino, and a

resident of Butuan City, after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That I am the same and identical person who

caused the execution of this affidavit;

2. That I am the father of FRANCIS S.

NEPUMUCENO, 17 years old, and an applicant of
scholarship in Butuan City and one of their
requirements is this affidavit;

3. That I hereby declare with all honesty and good

faith that I am not gainfully employed nor receiving
any pension or annuity nor have a business of his
own, hence I dependent wholly upon the support of
my husband;

4. That I am executing this affidavit to attest to the

truth of the foregoing facts and for whatever legal
purpose it may serve.
