AFFIDAVIT

I, RAIAN O. SUELTO, of legal age, Filipino, married and a resident of

Purok 10, Barangay Kapatagan, Laak, Davao de Oro, Philippines after having
been duly sworn to in accordance with law, depose and state:

1. That I am an applicant for a position of Farm Supervisor at

Cambodia with Pioneer Asia Manpower Inc.;

2. That I undergo medical examination at St. Peter-Paul Medical Clinic

Davao at J. Luna cor. J. Dela Cruz, Davao City, which is approved
and authorized by the Department of Health (DOH);

3. That St. Peter-Paul Medical Clinic-Davao issued a Medical

Certificate for Land-based Overseas Workers;

4. That I was declared FIT FOR DUTY;

5. That this is in connection for the processing and compliance of the

abovementioned application;

6. That I am executing this Affidavit to attest to the foregoing facts

and for all legal intents and purposes this may serve best.
