I, RICARDO TURLA ESTACIO, of legal age, married, Filipino, and a resident of

Purok 6, Brgy. 37-C Boulevard Trading, Davao City, after being duly sworn to in
accordance with law, do hereby depose and state;

1. That I am the owner of a RETAILER OF GENERAL MERCHANDISE (Sari-

Sari Store) located 991 purok 6, Trading Boulevard, Brgy. 37-D, Davao City;

2. That I hereby transfer the ownership and/or management of the said business
to my wife, BELLIE A. ESTACIO;

3. I am executing this Affidavit in good faith, for whatever legal purpose it

may serve and to attest to the veracity of the foregoing;
