AFFIDAVIT OF EXPLANATION

I, JOEMARIE L. VERDEPRADO, of legal age, married, Filipino, and a

resident of 214 Mars St., Victoria Heights Subd., Brgy. Angliongto, Lanang,
Davao City, after being duly sworn in accordance with law, hereby depose and
say THAT :

1. I am the proprietor and manager of CHEMVEST COMMERCIAL

TRADING with business address at Co Building, Damaso Suazo St.,
Davao City;

2. Our business is into wholesale and retail of laboratory, hospital, school

supplies, rescue equipment, and cleaning supplies;

3. Since 2016, our business has several contract agreement with the
Department of Education , Region XI for the supply of laboratory,
dental and medical supplies;

4. By reason thereof, I will not be able to submit the CAP Policy which is
one of the requirements on plan termination;

5. I am executing this affidavit in order to apprise CAP of the foregoing

facts and for whatever purpose it may serve.
