AFFIDAVIT OF CONSENT

I, DOMINADOR C. AQUINO of legal age, Filipino and a resident of Garapan, Saipan,

after having been duly sworn to in accordance with law hereby depose and say:

1. That I am the heir of BALDOMERO AQUINO - deceased, the register owner of a

parcel of land identified as Lot 1414, situated at Baybay Polong, Binmaley, Pangasinan,
containing an area of 1294 square meters, more or less declared under Tax Declaration
No. 16956;

2. That on proposed date BARBARA FLORES will construct their Residential House on
the portion of the above-mentioned property with our consent and conformity;

3. That the virtue of an affidavit we are giving a full consent and conformity regarding the
application of BARBARA FLORES for a building/electrical permits and zoning
clearance at the City Engineer’s Office/City Planning and Development Office,
Binmaley , Pangasinan;

4. That I am executing this affidavit to attest the truth of the foregoing statements and for
whatever legal purposes it may serve.
