AFFIDAVIT OF UNDERTAKING

I, ERIC E. CARVAJAL, of legal age, married, Filipino Citizen, with residence and
postal address at Daclan, Tublay, Benguet, Philippines, hereby depose, and state:

1. That I bought a 120 sq.m lot along Acop-Kapangan National Road, bearing Tax
Declaration no. 001-909111-A;

2. That I want to construct a building on the said lot which abuts a national road;

3. That I am aware that it is with the 30 m. radius of the national road;

4. That I am executing this affidavit to attest my promise to voluntarily remove/dismantle

whatever structure made thereto in case there will be a widening/concreting or any improvement
in the future to any portion beside my lot or any portion of my lot with the 30m radius without
any compensation from authorize government agency particularly the DPWH.

5. That I am executing this affidavit as a supplemental document to my request from the

DPWH for the clearance of my proposed project.
