AFFIDAVIT OF APPLICANT
I, EVELYN A. PIAS, of legal age, single, Filipino Citizen and a resident
of Km. 20 Cagui-ing, Atok, Benguet, Philippines, after having been duly
sworn to in accordance with law hereby depose and state:
1. That I am the niece of KEBENG DONATO an owner of a residential
house located at Km. 20 Cagui-ing, Atok, Benguet and a former
consumer of BENECO with old Account Number 9001-0078-02;
2. That sometime in 2008 the ELECTRIC CONNECTION was
disconnected by the BENECO for the reason that theres no
occupant in the house of Kebeng Donato;
3. That sometime in January, 2014 I decided to occupy the house of
my uncle through his authorization and now I intend to apply for reinstallation of Electricity at the said house;
4. That my uncle verbally authorizes me to reconnect the electricity,
however, I could not get written authorization from him because he
is in the far flung areas of Cebu;
5. That any problem that will arise in connection with the reconnection of electricity, the BENECO will be free from liability;
6. That I am executing this Affidavit to attest to the truth of the
foregoing facts and to support my application ELECTRIC
CONNECTION and for all legal intents and purposes it may serve.
IN WITNESS HEREOF, I have hereunto set my hand this 4 th day of
June, 2015 at Baguio City, Philippines.
