AFFIDAVIT
I, DARAMALLA JUVAMMA D/o. DARAMALLA RAMULU, aged 55 years Occ: Household, R/o.
Cherukupalli (V), Kethepally Mandal, Nalgonda District do hereby solemnly affirm and state
on oath as follows :-
1. That I am deponent herein, hence I know the following facts of this affidavit.
2. That I have married with Yesupadam S/o. Ramaiah R/o. Marruur in the year 1985,
and we do not have any children till the year 2000.
3. That due to the problem of having children my husband divorced with me before our
family elders and he got married with another in the year 2000.
4. That then after my husband got remarried in the year 2000, I am living along with my
mother and father since 20 years as individual women without family.
5. That my father was died in the year 2002 and mother was also died in the year 2017
by leaving me only person in our family.
6. That now I am applying for “Vontari Mahila Pension” which was giving by Telangana
State Government.
7. That I request the Mandal Parishad Development Officer, Kethepally to verify my
application and take necessary action in my favour in the interest of justice.
That the above information is true to the best of my knowledge and belief.
