AFFIDAVIT
I, SAMMY NGIGI KAMAU, of Post Office Box Number 4080 NAKURU, within the Republic of Kenya
do hereby make oath and solemnly declare as follows: -

1. THAT I am a male adult of sound mind and disposition and the holder of Identity Card Number
3629776. (Annexed is a copy of my Identity Card.)
2. THAT I know DANCAN KAMAU MUHOI(deceased) by dint of him being my late father, who passed
away on the 31st July, 1991.
3. THAT I know that before my father’s demise, he owned some shares at the EAST AFRICAN
BREWERIES LIMITED and KENYA BREWERIES LIMITED, which have since undergone
succession and a certificate of confirmation of grant issued to that effect vide NAKURU SUCCESSION
CAUSE NO. 153 OF 2009.(Annexed is a copy of certificate of confirmation of grant).
4. THAT I know that MUHOI KAMAU ALIAS DANCAN MUHOI KAMAU ALIAS KAMAU
MUHOI refer to one and the same person.

5. THAT the inconsistency in the names was an error on the part of the entity issuing the share certificates
but I affirm that all the three names refer to one and the same person and that is DANCAN KAMAU
MUHOI.
6. THAT further I wish to clarify that the difference in names is not meant to deceive or mislead but the
same is merely a human error that is excusable in the circumstances.
7. THAT what is deposed to herein above is true to the best of my knowledge, information and belief and
in conformity with the Oaths and Statutory Declarations Act.

2. THAT I am also in possession of a certificate of grant dated 8th May 2022 bearing my name as SAMUEL
NGIGI hence this affidavit.
3. THAT I make this application in support of my application to the concerned authorities that the name
SAMMY NGIGI GATIMU in my identity card and SAMUEL NGIGI in the certificate of confirmation
of grant refer to one and the same person

4. THAT further I wish to clarify that the difference in names is not meant to deceive or mislead but the
same is merely a human error that is excusable in the circumstances.
5. THAT what is deposed to herein above is true to the best of my knowledge, information and belief and
