1. THAT I am an adult female of sound mind and a holder of Republic of Kenya

Identification Card Number 27316473 and hence duly competent to swear this
Affidavit.

2. THAT I am known by the name JANE WAITHIRA as appears in my Birth

Certificate Number 1789003. (Annexed hereto is a copy of the said Birth Certificate)

3. THAT my names in my identity card appear as JANE WAITHIRA KIAGO.

(Annexed hereto is a copy of the said Identity Card.)

4. THAT the names appearing in the aforementioned Identity card and Birth Certificate
as JANE WAITHIRA and JANE WAITHIRA KIAGO respectively, are my names
and refer to one and the same person.

5. THAT I make this declaration in verification of my identity as referred to in my

Identity Card and the Birth Certificate.

6. THAT all the above facts are true to the best of my knowledge and belief.
