AFFIDAVIT OF NON-OPERATION

in accordance with law, hereby depose and state that:

1. I am the <Designation> of PHILIPPINE BANK OF

COMMUNICATIONS (the “Bank”), a commercial banking corporation
duly organized and existing under Philippine laws with principal office
address at PBCOM Tower 6795 Ayala Ave. cor V. A. Rufino Street, Makati
City, Philippines 1226.

2. The Bank no longer intends to operate its business

located at 35th Floor, Wynsum Corporate Plaza, Emerald Avenue,
Ortigas Center, Pasig City, as evidenced by the fact that the Bank has
not been occupying or using the said premises since _______________
2017.

3. By virtue of the reason mentioned in the immediately

preceding paragraph, we hereby request that the Bank be included in
the list of corporations whose barangay clearance shall no longer be
renewed by the Punong Barangay of San Antonio.

4. This Affidavit of Non-Operation is executed to attest to the

truth of the foregoing and for whatever legal purpose it may be worth.
