PETITION

COMES NOW, Petitioner, by himself and unto this Honorable

Court, most respectfully alleges THAT:

1. This is a Petition for Commission as Notary Public under

Rule III of the 2004 Rules on Notarial Practice (Supreme Court
Administrative Matter No. 02-8-13-SC), which took effect on 01
August 2004, and has been read and fully understood by the
Petitioner;

2. Petitioner is 32 years old, Filipino citizen, married, born and

is permanently residing at #553, Zone 6, Annafunan, Solana Cagayan
with mobile number 09274488226;

3. Petitioner is engaged in the private practice of his profession

with office address at Unit 102, Balai Balai Legaspi, Centro 21,
Tuguegarao City, Cagayan, where he will also be discharging his
functions as Notary Public which is within the territorial jurisdiction
of this Honorable Court;

4. Petitioner is a Member of the Integrated Bar of the

Philippines and has paid his annual membership fee evidenced by
IBP Official Receipt No. 2543543 dated August 25, 2021, copy of
which is hereto attached as Annex “A” to form integral part hereof;

5. Petitioner has likewise paid his Professional Tax for the year
2021, in the Province of Cagayan, under Official Receipt No. CGYN-
87687685 dated October 29, 2021, copy of which is hereto attached as
Annex “B” to form integral part hereof;
 6. Petitioner has never been convicted of a crime involving
moral turpitude and is a member of the Philippine Bar in good
standing duly authorized to practice law under the laws of the
Republic of the Philippines, as evidenced by the following:

i. Certificate of Membership in the

Philippine Bar issued by the Office of the Bar
Confidant of the Supreme Court of the
Philippines on September 2, 2021, original copy
of which is hereto attached as Annex “C”;

ii. Certification issued by the Office of

the Bar Confidant of the Supreme Court of the
Philippines dated September 2, 2021 to the
effect that Petitioner has no pending
administrative case against him filed with the
said office. Original copy of the same is hereto
attached as Annex “D";

iii. Certification issued by the

Integrated Bar of the Philippines dated
August 19, 2021 to the effect that petitioner
has no pending administrative/disbarment
case filed against him. Original copy of the same
is hereto attached as Annex “E”;

iv. Certification issued by the Cagayan

Chapter of the Integrated Bar of the
Philippines dated October 21, 2021 to the
effect that petitioner is a member of the said
chapter in good standing. Original copy of the
same is hereto attached as Annex “F-1” and "F-
2";

v. Certificate of Compliance No. VI-

7657657 dated February 27, 2019 issued by the
Supreme Court - Mandatory Continuing
Legal Education Office to the effect that
petitioner has complied with the required 36
credit units of MCLE, Compliance No. VI,
 under Bar Matter No. 850. Copy of the same is
hereto attached as Annex “G”;
7. Petitioner submits herewith and makes an integral part
hereof as Annex “H” the mark, images or impression of his Notarial
Seal prepared in accordance with Section 2, Rule VII of the 2004 Rules
on Notarial Practice for the approval of the Honorable Court;

8. Attached herewith as Annex “I” and made an integral part

of this petition is petitioner’s duly signed three (3) 2x2 size, non-
retouched colored photographs taken within thirty (30) days from the
date of filing of the instant Petition;

9. Furthermore, Petitioner submits herewith as Annexes “J”

and “K” respectively and made integral parts hereof, the photograph
of his office and the sketch of its location;

10. Petitioner also submits his specimen signatures which is

hereto attached as Annex “L” to form integral part hereof;

11. Petitioner also submits his Authority to Practice issued by

the Central Business Philippines which is hereto attached as Annex
“M” to form integral part hereof; and

12. Finally, simultaneous with the filing of this petition, filing

fees and other lawful fees shall be paid by the petitioner to the
Cashier of this Honorable Court evidenced by official receipts, copies
of which are hereto attached as Annexes “N” to form integral parts hereof.

PRAYER

WHEREFORE, in view of the foregoing, it is respectfully

prayed that Petitioner’s application for commission as Notary Public
for and within the territorial jurisdiction of the Province of Cagayan,
be granted for the years 2022-2023;
Other reliefs just and equitable under the premises are likewise
prayed for.
