CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF SERVICE is made and entered into by and between the SANGGUNIANG BAYAN OF THE
MUNICIPALITY OF VALLADOLID , Province of Negros Occidental, with office address at Zarandin Street, Barangay
Poblacion, Valladolid , Negros Occidental ,represented herein by its Municipal Vice Mayor ,HONORABLE ROEL P.
YOGORE ,hereinafter referred to as the “FIRST PARTY;

And

FRANCISLYNE C. NAPUGOT, of legal age, Filipino, married and a resident of Barangay Poblacion, Valladolid, Negros
Occidental, Philippines, hereinafter referred to as the ”SECOND PARTY”

WITNESSETH THAT:

1.The FIRST PARTY employs the services of the SECOND PARTY who shall carry out works not performed by the
regular personnel of the First Party:
2.The SECOND PARTY has signified his intention to which the FIRST PARTY has accepted, to provide the services
needed by the letter;
3.The SECOND PARTY possesses the education , experience ,skills and is qualified to perform the job as required
herein , to wit;

a. Under the office of the Municipal Vice -Mayor ,and the Office of the Sangguniang ,the Second Party
shall render services to encode all data resulting from the official transactions of the aforementioned
office;

b. To update the encoding of Resolutions and Ordinances ;and

c. To archive other important documents under the office of the Municipal Vice-Mayor and the
Sanggunian, and

d. To perform such other functions as maybe assigned by the Municipal Vice-Mayor .

4. In view hereof, the SECOND PARTY is hereby contracted as DATA ENCODER from July 1,2023-Dec 31,2023, in
consideration of the monthly rate of EIGHT THOUSAND PESOS (P 8,000.00) to be paid on a monthly basis , and
chargeable against the General Services of the Vice-Mayor’s office;

5.The SECOND PARTY shall perform work at a time and schedule to be agreed upon by both parties, and

6.It is understood that this contract does not create an employer-employee relationship between the First Party
and the Second Party;

7.That the services rendered hereunder are not considered and will not be accredited as a government services,
and that the Second Party is not entitled to benefits enjoyed by the regular personnel of the First Party.

IN WITNESS WHWREOF, both parties have hereunto set their hands this ___ day of July,2023 at Valladolid ,Negros
Occidental, Philippines.
