CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This Contract of Service entered into by and between the Local Government of Bula,
Camarines Sur, represented herein by Municipal Mayor Amelita A. Ibasco with residence and
postal address at Bula, Camarines Sur, hereinafter referred to as the “First Party” and the Holy
Rosary Minor Seminary, represented herein by Rev. Fr. Jay Jacinto, of legal age, Filipino, with
residence and postal address at Naga City hereinafter referred to as the “Second Party”.

WITNESSETH:

1 . That the First Party, in pursuance to Municipal Ordinance No. 2015-02-022 of Bula,
Creating the Municipal Youth Officials, is in need of the services of the Second Party to
conduct qualifying examination and interview to the applicants as part of the selection process
of the MYO of Bula, Camarines Sur.
2. That the Second Party has signified the intention of which the Second Party has accepted to
provide the services indicated below:
a. conduct intensive examination
b. conduct interview
3. That in view hereof, both parties are hereby contracted to each other to perform the aforesaid
services in consideration of the amount of TEN THOUSAND PESOS (P10,000.00) only
broken down as follows:
a. Conduct of Examination -- P 5,000.00
b. Conduct of Interview -- P 5,000.00
4. That it is understood that this Contract does not create employer-employee relationship
between the First Party and the Second Party.
5. That the services rendered are not considered and will not be considered as government
service, and that the latter is not entitled to benefits enjoyed by the regular personnel of the
First Party.

IN WITNESS WHEREOF, the parties hereunto set their hands this ___ day of
