EXIT MEMORANDUM OF AGREEMENT

This Memorandum of Agreement made and entered into this 30th day of August 2019 by
and between:

The Department of Social Welfare and Development, a national agency of the Republic
of the Philippines, with office address at Batasan Pambansa Complex, Diliman, Quezon
City herein represented by _______________________ in his capacity as Regional
Director of Field Office CAR, with office address at # 40 North Drive, Baguio City,
hereinafter referred as the DSWD;

and

The Municipality of ___________, a municipal corporation of the Republic of the Philippines


created under existing laws with principal address at _________________, Province of
_______________, hereinafter represented by ____________________ in his capacity as
Municipal Mayor, and hereinafter referred to as the Municipality;

Witnesseth that:

WHEREAS, the government of the Republic of the Philippines through the DSWD
implemented the Kapit Bisig Laban sa Kahirapan Comprehensive and Integrated Delivery
of Social Services-National Community Driven Development Program (KALAHI-CIDSS-
NCDDP) Program, herein referred as the “Project”, in the municipality through the
assistance of the World Bank;
WHEREAS, the Project aimed to empower local communities through improved
participation in local governance and involvement in the preparation and implementation of
poverty reduction projects;
WHEREAS, the Project called for the active involvement of the local government unit in the
provision of local counterparts and other technical assistance requirements to support
prioritized barangay sub-projects that address local poverty conditions;
WHEREAS, the program timeline for project implementation has already ended and based
on the final sub-project validation conducted there are 21 sub-projects declared completed
from Cycle 1 to cycle 4 and turned over to the barangay and its Operation and Maintenance
group/association;

WHEREAS, in order to sustain the gains and benefits derived from implementing
Community Driven Development through the KALAHI-CIDSS: NCDDP, the MIAC, BLGUs,
Community Volunteers and other stakeholders formulated a Sustainability Plan that
contains strategies for the adoption and integration of practices and experiences in the local
planning process of the LGU.
WHEREAS, it is imperative for the Municipal Local Government Unit to ensure the
implementation, conduct of assessment and monitoring of the sustainability plan.
WHEREAS, the Municipality have expressed its commitment to provide necessary support
for the implementation of the sustainability plan towards a more participatory, transparent
and accountable local governance.
WHEREAS, the DSWD, through the regional Field Office, expressed its interest to continue
providing technical support to the municipality in order to ensure the completeness and
closing activities of the project;
WHEREAS, ALL PARTIES have decided and formally bind themselves to continue
partnership efforts in ensuring the implementation of the sustainability plan and the benefits
it will provide to the target communities;

Article 1- Roles and Responsibilities of the Parties

1. DSWD

The DSWD through the regional field office shall;

1.1 Assign focal persons for Social Process, Engineering and Finance who shall
provide continuing technical assistance to the municipality upon request and/or
the need arises in the areas of:
1.1.1 closure of account or fund and other closing requirements for
completed sub-projects;
1.1.2 organizational strengthening which includes recognition, registration
and accreditation of the Operation and Maintenance group;
1.1.3 conduct of the Sustainability Evaluation to ensure functionality of the
completed subprojects;
1.1.4 implementation of the Municipal Sustainability Plan; and
1.2 Inform and invite the municipalities on whatever Programs, Projects and Activities
relative to the CDD programs.

2. Municipality

2.1 Continue the functionality of the Municipal Coordinating Team who shall be
tasked to assist the barangay and municipality in the completion and closure
activities of all sub-project of the communities;
2.2 Continue the functionality of the Municipal Inter-Agency Committee to assist in
providing technical assistance to the barangay and municipality for the closure
activities;
2.3 Provide support in terms of Cash and In Kind for the repair and maintenance of
the sub-project to ensure the sustainability of benefits to the community;
2.4 Designate a focal person for the strengthening and implementation of Operation
and Maintenance plans and programs at the barangay level and provide financial
support for such implementation;
2.5 Provide support to relevant Operation and Maintenance Ordinances enacted and
implemented by the barangays to be monitored by the concerned MLGU staff;
2.6 Provide technical assistance, support, passing of resolution, ordinances and
creation of technical team/committee for effective implementation of sustainability
plan;
2.7 Ensure that the affected barangays have been consulted and agree to do the
following roles and responsibilities as enumerated below through issuance of
Barangay Assembly resolution;

2.7.a Take the lead in ensuring the supervision of any remaining deliverables
to complete the closing of requirements;

2.7.b Provide support to relevant Operation and Maintenance Ordinances


proposed by the O and M group and barangay assembly through
enactment and facilitate approval by the Sangguniang Bayan;

2.7.c Ensure that completed sub-project is booked as asset of the barangay


and registered in the Book of Registry; and

2.8 Continue to support the current and future DSWD programs, projects and
activities.

Article II- Effectivity and Amendments

This Memorandum of Agreement shall take effect upon signing. All provisions stated in this
MOA shall continuously complied with by all parties
