To Whom It May Concern:

This is to CERTIFY that the emergency repair/purchase covered by this Purchase

Request was made in view of the following:

a. That the need for the item repair/purchase exceptionally urgent to avoid serious
detriment to the public service;

b. That there was no material time procure through public bidding due to the urgency of
the requirement;

c. That the prices of the supplies procured directly were lowest obtainable in the market
at the time of purchase; and

d. That the parts/repair/purchase availed were not available or insufficient in stock in the
storeroom of the office.
