LANDBANK LINKBIZ PORTAL FACILITY
“LINKBIZ PORTAL”

Whereas, on ____________ 20___, the SP of _______________ has approved Resolution No. ____, s. 20___
authorizing (Please state name and position of responsible officer to be designated) to the following:

1. Subscribe to the Land Bank of the Philippines’ (LANDBANK’s) internet banking facility known as
“weAccess”, and online payment system known as “Link.BizPortal”,

2. Designate authorized signatory to enroll and designate authorized user/s in the LANDBANK weAccess.

3. Designate authorized signatory to enroll and designate authorized user/s in the LANDBANK
Link.BizPortal Merchant Payment Inquiry.

NOW, THEREFORE, as it is hereby Resolved, authorizing (Please state name and position of responsible officer
to be designated) to:

1. execute/sign all documents necessary to subscribe to LANDBANK’s “weAccess” and “Link.BizPortal;

2. designate/authorize personnel to act as “maker” or “authorizer” for the company’s access to

LANDBANK’s weAccess banking facility; and

3. designate/authorize personnel to act as “user” for the school’s access to LANDBANK’s Merchant
Payment Inquiry module;

4. amend/add/delete enrolled deposit account/s as source and/or destination account/s, access rights to
the weAccess modules, authorization rules and/or any information on the original and succeeding
enrollments; and

And that the same is hereby confirmed and ratified;

THAT, this is an addendum to the signatories of the submitted resolution for account opening;

Resolved finally, to furnish copies of this Resolution all concerned for their information and appropriate
action.
