AFFIDAVIT OF UNDERTAKING

I, DIVINE B. PURISIMA, of legal age, Filipino, married, with

residential and postal address at No. 069C Purok 27, San Carlos Heights,
Baguio City, Philippines, after having been sworn in accordance with law,
hereby deposes and says that:

1. I am applying for a water connection at Baguio Water District

to be connected to the above-mentioned property;

2. The water connection will traverse through my neighbor’s land

for the connection to reach my property;

3. Should the lot owner claim ownership over the land which my
water connection traversed, I am willing to disconnect the water services
from Baguio Water District;

4. In connection with this, I will not hold the BAGUIO WATER

DISTRTICT (BWD) liable;

5. I further undertake that I will not complain should the water

connection be removed in the future;

6. I am executing this Affidavit to attest to the truth of the

foregoing facts, and for all legal intents and purposes it may serve.
