AFFIDAVIT OF RELATIONSHIP

I, SIZHUO YU, Chinese Citizen, married, of legal age with resident address
at 15JK Bellagio No. 3 Forbes Town Town Road, Bonifacio Global City, Taguig
after having been duly sworn in accordance with law, hereby depose and state:

1. That I am the father of Xin Yu, eight years (8) old and a Chinese Citizen.
2. That to prove our relationship, attach herewith is a copy of my passport and
Xin Yu’s Passport together with the following documents:
a) Sizhuo Yu’s Passport No. G54051499/ Xin Yu’s Passport No. 89878121
b) Xin Yu’s Birth Certificate April 3, 2011;
c) Letter of Certified Translation of the Birth Certificate
3. That Xin Yu was born in the Hunan, People’s Republic of China on April 17,
2011.
4. As such, I am executing this Affidavit to attest to the truthfulness of the
foregoing and for whatever legal purposes that this may serve.
