AFFIDAVIT

I, ROSALYN D. FAN, of legal age, Filipino, married with residence and

postal address at No. 17 PRK 28, Upper Irisan, Baguio City, after having
been sworn in accordance with law, hereby deposes and says that:

1. I am currently processing my application as an English Teacher

to work abroad;

2. Among the requirements needed for the said job application

are necessary trainings and certificates related to teaching;

3. On 10 September 2019, I was issued a Certificate of

Completion by TONI VANILLI ENTERPRISE (TVE) Coaching Enterprise,
Baguio City, Philippines that I have enrolled and completed 120 hours of
training for TEACHING ENGLISH AS A FOREIGN LANGUAGE (TEFL);

4. As proof of the above-said training, a copy of the Certificate of

Completion of the course: TEACHING ENGLISH AS A FOREIGN LANGUAGE
(TEFL) issued to me by TONI VANILLI ENTERPRISE (TVE) Coaching
Enterprise is hereto attached;

5. I executed this affidavit to attest to the truth and veracity of all

the foregoing facts and for all other legal intents and purposes.
