AFFIDAVIT OF LOSS

I, JOY IHINRERE OLORUNDA, of legal age, a Nigerian citizen, single, with

residential and postal address at Lot 4, Bakakeng Road, Bakakeng Norte/Sur,
Baguio City, after having been sworn in accordance with law, hereby deposes
and says that:

1. I am the user of a pre-paid Globe SIMCARD with number 0906-450-

1955;

2. On November 14, 2022, I brought my phone with me to school

using the said Globe SIMCARD;

3. However, in the afternoon when I got home, I looked for my phone,

then, I discovered that it was no longer in my bag where I placed it;

4. I looked for the phone realized that I have misplaced it;

5. I have exerted all efforts to find the phone with pre-paid Globe
SIMCARD with number 09064501955, but to no avail. I hereby declare that the
same is lost and beyond recovery;

6. I executed this affidavit to attest to the truth and veracity of all the
foregoing facts and for all other legal intents and purposes.
