AFFIDAVIT OF LOSS

I, MA. LAARNI MAY LAGUARTILLA CATAMCO, of legal age, single, residing at

Pusok, Lapu-Lapu City, Philippines, after being sworn in accordance with law, depose
and say:

1. That I am a bona fide student of Cebu Technological University;

2. That I was duly issued a School Identification by the abovementioned school;

3. That sometime on August 15, 2022, around 6 p.m, I was strolling inside the mall
and I kept my School ID inside my wallet for safe keeping;

4. However, after arriving home, I noticed that my School ID was already missing
from my wallet wherein the abovementioned item was kept and it might have
fallen out of my wallet;

5. That I directly went back to my route and diligently exerted efforts to locate
the said School ID, but despite a diligent search it could not be found, hence
the same is deemed lost and at a point beyond recovery;

6. That I am executing this affidavit to attest to the truth and veracity of the
foregoing facts so that I may be issued a new School ID in lieu of the lost one,
and for whatever legal purpose this may serve.

IN WITNESS WHEREFORE, We have hereunto affixed our signature this 30th day of
August, 2022, at Cebu City, Philippines.
