AFFIDAVIT OF SOLO PARENT

I, MARIFE C. ENDAB, a Filipino Citizen of legal age, single, and residing at Crossing,
Marigondon, Lapu-Lapu City, Cebu, Philippines, after being sworn to in accordance
with law, depose and say:
1. That I and the biological father of our child got separated. In fact our
relationship was terminated and have never communicated ever since the
separation;

2. I was thereafter left alone with the sole responsibility of parenthood, and I am
solely providing for the needs of my child;

3. That my child, JOHN MARKY ENDAB is already under my custody.

4. I am executing this Affidavit to attest that I am not cohabitating with another

partner or co-parent and has sole parental care and support of my child, and
for whatever purpose this may serve.

IN WITNESS WHEREFORE, I have hereunto affixed my signature this 12 th day of
