AFFIDAVIT OF LOSS

I, REINITA IGOY BOJOS, of legal age, single, residing at Ibapu, Mactan Lapu-Lapu City,
Philippines, after being sworn in accordance with law, depose and say:

1. That I am the caretaker with an Original Certificate Title (OCT) number 2019000004 owned
by my Grandmother namely Monica Godinez issued by the Registry of Deeds;

2. That I kept the above said item inside the cabinet in our house for safe-keeping;

3. However, the above-mentioned item was lost in my possession when the typhoon Odette
destroyed our area last December 16, 2021;

4. That despite diligent search and effort exerted to locate the said Orginal Certificate Title
(OCT), but the same proved futile; hence, the same should now be considered lost;

5. That the abovesaid item has not been encumbered, sold, pledge, assigned, alienated,
transferred, or give as collateral for any debt or obligation to or in favor of any person or entity;

6. I am executing the instant Affidavit to attest the veracity of the foregoing statements and for
the issuance of a new above-mentioned item in lieu of the lost one.
