AFFIDAVIT OF LOSS

I, VIRGINIA CABULLO BACANI, Filipino, of legal age, married and residing at

174 Barangay Biga II, Silang, Cavite, after having been duly sworn to in accordance with
law do hereby depose and say that:

1. I am a licensed Professional Teacher and likewise holder of a PRC

License with registration number 0492615 issued to me by Professional Regulation
Commission (PRC);

2. I usually keep my said PRC license in my wallet for safekeeping;

3. However, on October 2, 2020, I discovered that my said license was

missing;

4. Despite all diligent efforts exerted to locate my PRC license but to no avail
and nowhere to be found;

3. I executed this affidavit to attest to the truth and veracity of the foregoing
statements; to request proper authorities concerned to grant me a new PRC License in
lieu of the lost one and for all legal intents and purposes this may serve.

License issued to me by Professional Regulation Commission (PRC);

6. I usually keep my said PRC license in my wallet for safekeeping;

7. However, on March 6, 2020, I discovered that my said license was

missing;

8. Despite all diligent efforts exerted to locate my PRC license but to no avail
and nowhere to be found;

3. I executed this affidavit to attest to the truth and veracity of the foregoing
statements and for all legal intents and purposes this may serve.
