AFFIDAVIT OF LOSS

I, Gaano Revie Sawadan, of legal age, Filipino, single and a resident of New
Lucban extension rimando road Baguio city, after having sworn in accordance
with law, hereby depose and say:

1. I am the holder of a school identification card issued by Baguio central

university;
2. That I used to put said identification card inside my wallet;
3. That sometimes on February 24, 2021, when I arrived boarding house from
school, I discovered that my school ID was no longer in my wallet where I
used to place it;
4. That despite diligent search and efforts to locate the said identification
card, I could not find the same such that I now believe that school ID card
is now lost beyond recovery;
5. That said ID card was not confiscated by the authorities or used in violation
of the law;
6. That I am executing this affidavit to attest to the truth of the foregoing facts
and to request for a replacement of said school ID card from Baguio central
university.

Pinukpuk Tabuk city Kalinga, after having duly sworn to in accordance with
law hereby depose and state:

1. I am the complaining witness for serious physical injury against

Dawagan Gerald in the case entitled “people of the Philippines versus
Dawagan Gerald”, criminal case No. 88888, Regional trial court, Branch
25, 2nd Judicial Region, Bulanao, Tabuk city, kalinga.
2. After my sober and soul searching assessment and analysis of the
incident, I have realized that because I was not wearing my eyeglasses
and it was dark, I can not point out, without a doubt the accused or any
other person who inflicted haram against me.
3. Since I could not state with certainly and without doubt the liability of
Dawagan Gerald, in fairness to him, I am permanently withdrawing my
complaint against him. I clear him of whatever responsibility or liability
to me.
4. I hereby inform the city prosecutor of Tabuk that I am withdrawing my
complaint for serious physical injuries in criminal case No. 88888
entitled people of the Philippines vs. Dawagan Gerald, RTC, Brach No.25
2nd Judicial Region, Bulanao Tabuk city Kalinga.
5. I likewise request the RTC, Branch No. 25 2nd judicial Region, Bulanao
Tabuk city Kalinga to dismiss with prejudice the said criminal case.
