For Notarial Commission

The undersigned petitioner, Jun Bill M. Cercado, unto

this Honorable Court respectfully states that:

1. He is a Filipino citizen, twenty-four (24) years old

having been born on August 19, 1996 in Calasiao,
Pangasinan, single, a resident of #164 Honeymoon Road,
Baguio City, and with office address at Room 105, Patria de
Baguio, Session Road, Baguio City, Philippines with telephone
no. (074) 619-2044;

2. He has satisfactorily completed the required course

in Juris Doctor from the School of Law in Saint Louis
University, Baguio City, Philippines on May 25, 2019;

3. He passed the 2019 Bar Examinations and was

admitted to the Philippine Bar on July 25, 2020 with Supreme
Court Roll of Attorneys No. 111888. A copy of his Certificate of
Admission to the Philippine Bar and his Certificate of
Oathtaking as a member of the Integrated Bar of the
Philippines issued by Office of the Bar Confidant, Supreme
Court of the Philippines, are attached herewith as Annex “A”
and “A-1” and made an integral part in this petition;

4. He is a member of the Philippine Bar in good

standing as evidenced by the attached Certification issued by

the Office of the Bar Confidant and Certification issued by the
Integrated Bar of the Philippines Baguio-Benguet Chapter
marked as Annexes “B” and “C” respectively;

5. Too, he has already paid his annual dues as a

member of the Integrated Bar of the Philippines and his
professional tax receipt, both for 2021, copies of the receipts of
his payment to the Integrated Bar of the Philippines and the
Office of the City Treasurer of Baguio are herewith attached as
Annexes “D” and “E” respectively;

6. He has never been charged or convicted of any

crime involving moral turpitude as evidenced by the clearance
issued by the National Bureau of Investigation and another
clearance issued by the Regional Trial Court of Baguio City. A
copy of the NBI Clearance and the RTC Clearance issued by
the Office of the Clerk of Court of Baguio City are herewith
attached as Annexes “F” and “G” respectively;

7. Petitioner attached three (3) passport-size and

colored photographs as required by the 2004 Notarial Rules
attached as Annex “H”;

8. Attached hereto are three specimens of his official

signature and three (3) impressions of his dry seal which are
attached as Annex “I.”

9. Petitioner had paid the filing fee of the petition, as

demanded by the 2004 Rules on Notarial Practice, evidenced
by a copy of Official Receipt No. 456789 issued by the Office of
the Clerk of Court of the Regional Trial Court of Baguio City,
attached herewith and marked as Annex “J”;

10. He has not yet applied nor been commissioned as

Notary Public in any jurisdiction and that he desires to be
commissioned as a Notary Public for and in the City of Baguio
and is willing to abide by the 2004 Rules on Notarial Practice;
and,

11. He possesses all the qualifications and none of the

disqualifications for the office of the Notary Public;

 WHEREFORE, premises duly considered, it is most
respectfully prayed that the undersigned be appointed as a
Notary Public for and in the City of Baguio effective on the
date of the approval of this Petition, until December 31, 2022.

Such other reliefs, just and equitable under the

premises, are likewise being prayed for.

provide a complete statement of the present status thereof;
and

3. If I should thereafter learn that the same or similar

action or claim has been filed or is pending, I shall report the
fact within five (5) calendar days therefrom to the court
wherein my aforesaid complaint or initiatory pleading has
been filed.
