AFFIDAVIT OF AUTHENTICITY

I, JOMER CABANILLA ENRIQUEZ, of legal age, Filipino citizen and with residence and
postal address at Block K-13, Lot 6, San Antonio de Padua II, City of Dasmariñas, Cavite,
Philippines, after having been duly sworn to in accordance with law hereby depose and
say, that:

1. That I am the Barangay Administrator of Barangay San Antonio de Padua II, City
of Dasmariñas, Cavite;

2. That on November 24, 2020 at around 5:00 o’clock in the afternoon, children were
playing near barangay hall Barangay San Antonio de Padua II, City of
Dasmariñas, Cavite and a certain Dante Masculino arrives with his pedicab
(Sidecar) and told them to get down from his sidecar. The children immediately
get down from the side car but all of a sudden he grabbed the child named John
Felix O. Royeras and pushed/thrown him resulting the child fell in the ground;

3. That I have extracted the incident from the cctv footage and copied and saved it
to re-writable CD and hereby attest that it is genuine and authentic;

4. That I hold myself liable for perjury, falsehood, misrepresentation or omission or

any act of dishonesty for any fraudulent, false, and tampered documents that I
have submitted;

5. I am executing this Affidavit to attest to the truthfulness of the foregoing facts

and for the authenticity and veracity of the above-mentioned incident and
document submitted and for whatever legal purposes and intents the same may
serve best;
