PETITION
COMES NOW, Petitioner, through this Honorable Board, respectfully alleges:

1. MELODEN D. BULANTA is the lawful owner of Lot 913, Pls-749 situated in the

Barangay Poblacion, Municipality of Sumilao, Province of Bukidnon, Island of

Mindanao;

2. That the lawful ownership of the petitioner is evidenced by the Photocopy of the Original

Certificate of Title No. AO-5580 issued by the Registry of Deeds of Malaybalay City

Bukidnon, marked as Annex “A”;

3. That the name of the petitioner in the Original Certificate of Title was erroneously

recorded as “MELODING D. BULANTA” instead of “MELODEN D. BULANTA”

per Photocopy of her Certificate of Live of Birth , marked as Annex “B”;

4. That the petitioner at the time the Original Certificate of Title was issued was still single;

5. That “MELODING D. BULANTA” and “MELODEN D. BULANTA” refers to one

and the same person based on the affidavit of two disinterested persons, marked as

Annex “C”;

6. That the petitioner is now married under the name of “MELODEN BULANTA

GALPO” as evidenced by the photocopy of her voter certification and a photocopy of

her community tax certificate, both marked as Annex “D” and Annex “E”.
 PRAYER

WHEREFORE, it is respectfully prayed that the petition be granted and the Registry of
Deeds be ordered to correct the entry of the name of the petitioner in the Original Certificate of
Title.

Other just and equitable relief and remedies are likewise prayed for.

That, I, FATIMA ANIQA T. MACARAMBON, Filipino, of legal age, and a resident of

CAGAYAN DE ORO CITY, after having been duly sworn to an oath in accordance with law hereby
depose and say:

1. That I am the Petitioner of the above-entitled petition for Inclusion;

2. That I have read and understood the contents thereof;
3. That I have caused the preparation of the same;
4. That the allegation therein are true and correct based on my personal knowledge
and authentic records;
5. That I have not instituted similar action to my present case with the Supreme Court,
Court of Appeals, Tribunal, Board or any Agency;
6. That I have no knowledge of any similar action pending terminated in any institution
mentioned above; and
7. That if I should eventually knew that there is similar action pending in the above-
mentioned offices within five (5) days I will inform this Honorable Board where my
petition is originally instituted.
