AFFIDAVIT OF CONFIRMATION OF SUBDIVISION

That I, BERNARD P. MENDOZA, widower, Filipino Citizen and a resident of

Nay-en, Ampucao, Itogon, Benguet and National Power Corporation (NAPOCOR),
a government owned and controlled corporation created by and existing under
Republic Act No. 6395, as amended, with principal address at NPC Power
Complex, BIR Road corner Quezon Avenue, Diliman, Quezon City herein
represented by its Vice President from the Office of the Legal Counsel, Atty.
MELCHOR P. RIDULME, after having been duly sworn to an oath in accordance
with law, do hereby depose and say:

That BERNARD P. MENDOZA and the NAPOCOR are the co-owners of that
real property identified as Lot No. Plan F-62287 located at San Felipe West, San
Nicolas, Pangasinan containing an area of 60,266 square meters covered by
Transfer Certificate of Title No. T-25684.

That the NAPOCOR thru its contractor, VIRCON SURVEY SERVICES have
caused the survey of the above described property into three (3) lots which are
approved as follows:

TO THE NATIONAL POWER CORPORATION, with an area of 2,256

square meters, with respect to Lot 2 (Road);

TO BERNARD B. MENDOZA, with an area of 45,740 and 12,270

square meters, with respect of Lots 1 and 3, respectively.

That we confirm the foregoing subdivision survey to be true and correct to

the best of our knowledge and information.

That we are executing this affidavit in order to indicate in details our

ownership over our individual lots as appearing above and/or to correct any
erroneous entry in the title relative to our individual lots.

IN WITNESS WHEREOF, we have hereunto set our hands this ______ day
