AFFIDAVIT OF CONFORMITY

a resident of ______________, after being duly sworn to in accordance
with law, hereby depose and say, that:

1. I am the owner and possessor of the following parcels of land by

virtue of an Deed of Absolute Sale, to wit:

a. Lot No. 2 being a portion of Lot No. _____, PLS-____,

covered by TCT Nos. _____ with an area of Seven Thousand
Four Hundred Fifty Five (7,455) square meters.
b. Lot No. 3 being a portion of Lot No. _____, PLS-____,
covered by TCT Nos. T-_____ with an area of Eight Hundred
Eight Two (882) square meters.

2. As a result of the survey, the Geodetic Engineer prepared

Subdivision Plan ______________.

3. I, therefore, signify my conformity to the result of the subdivision

survey as mentioned before.

4. This Affidavit is being executed freely, voluntarily and intelligently

in order to attest to the truth of the foregoing to declare my
conformity to the subdivision of the Lot No. _____________ in
the manner indicated above, and in compliance with the
requirements prescribed by the Department of Environment and
Natural Resources.

Affiant further sayeth naught.
