AFFIDAVIT OF GUARDIANSHIP

I, LEOPOLDO S. BINAYUG, of legal age, married, Filipino and a

resident of Calamagui, Solana, Cagayan, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. I am the father of GEMALYN BINAYUG MANALILI, an Overseas

Filipino Worker in Hongkong.

2. Gemalyn B. Manalili have a minor child whose name is MICHAELA

B. MANALILI.

3. I am the grandfather and guardian of the minor Michaela B. Manalili.

4. Michaela B. Manalili is under my care and custody.

5. I am not an imbecile, insane, a vagrant or a vicious or a habitual

drunkard or a habitual criminal, and have not abandoned, neglected or
refused to support the said minor or caused her to commit offenses
against the law.

6. I am executing this Affidavit to attest to the truth of all the foregoing

statements and for whatever legal purpose it may serve.

Affiant further sayeth naught.
