I, DOLSON R. ALINGOD of legal age, Single, Married, and resident of Brgy

Quinagabian, Sta. Maria, Isabela after being duly sworn to in accordance with law,
depose and state that:

1. I am the guardian of Adam Limuel Limbauan who was born on October 08,
2009 , Princess Stephanie Limbauan who was born on November 22, 2013 and
Terrence Josh Limbauan who was born on March 17, 2015 ;

2. The said minors are under my care and custody;

3. I have not been diagnosed nor committed to a hospital or institution for imbecile,
insane, a vagrant or vicious person. I am not a habitual drunkard or a habitual
criminal, and have not abandoned, neglected or refused to support said minors
or caused them to commit offenses against the law;

4. I am executing this affidavit to confirm and affirm the foregoing facts and to
prove that I am the legal guardian of the above-named minors, and for whatever
legal purpose it may serve.

Sur, San, Pablo Isabela;

2. The said minor is under my care and custody;

presently residing at Aggub, Cabagan, Isabela, after having been duly sworn to
inaccordance with law, do hereby depose and say:

1. That I am the guardian of CRISTOFF A, BALABBO, was born on

December 26, 2011, who have an interest in financial assistance offered by the Overseas
Workers Welfare Administration (OWWA) and for whatever legal intent:

2. That the above mentioned minor is under my care and custody; 

3. That I am not an imbecile, insane, a vagrant or a vicious person or

a habitual drunkard or a habitual criminal, and have not abandoned, neglected or
refused to support said minor or caused her to commit offenses against the law. 

4. That I am executing this affidavit to attest to the truth of all the foregoing
statements and for whatever legal purpose it may serve.
