sworn to in accordance with law, do hereby depose and say that:

1. I am the legal owner of a motorcycle, more particularly described as follows:

Topline Inc located at Ilog Pugad National Road Brgy. San Juan Taytay, Rizal;

3. That after purchase, I have attached a side car to said motorcycle; and

4. That I am attesting as to the truth of the foregoing, specifically for the

purpose of including the fact of said attachment of side car to the
motorcycles initial LTO registration, and for whatever legal purpose that this
may serve.
