AFFIDAVIT

I, CARIDAD MASIGAN, of legal age, Filipino citizen, and residing at Poblacion

I, St. Maria, Isabela, after having been duly sworn to in accordance with law, do
hereby depose and state THAT:

1. I am the same person executing this affidavit;

2. I am the owner of a parcel of land with Lot No. 353 Ts-2-02-000001 and
with an area of 1.057 sq.m. situated in Poblacion II, Sta. Maria, Isabela;

3. The said lot was subject for application of title before the PENRO – Cabagan,
Isabela. The said office informed us that the application was granted.
Attached hereto is the Transmittal Report submitted by PENRO to the
Register of Deeds (ROD) – Ilagan, Isabela;

4. That I never received a copy of the title of the said parcel of land from the
ROD because the said office was razed by fire, including the said office

5. I am executing the foregoing affidavit in order to attest to the veracity and

truthfulness of the above facts and for whatever legal purpose it may serve.
